WHAT IS A CASE MANAGER?

 

A Case Manager is a professional who coordinates a comprehensive package of care and rehabilitation for a person who has a significant disability. A Case Manager is normally recruited from a number of different professional and clinical backgrounds, such as Psychology, Nursing, Occupational Therapy, Social Work, Physiotherapy and other health related professions. Case Managers are regulated through registration by their own professional bodies.

 

WHAT IS CASE MANAGEMENT?

 

Case Management is a collaborative process which assesses, plans, implement, coordinates, monitors and evaluates the options and services required to meet an individual's health, care and vocational needs, using communication and available resources. We have a team of experienced and knowledgeable Case Managers to provide better outcomes for clients. We support clients throughout the rehabilitation journey, facilitating and coordinating all aspects of the recovery and rehabilitation..

 

WHAT DOES A CASE MANAGER DO?

 

The Case Manager will carry out an initial assessment of the client’s needs with the client and their family then produce a written report, goals and management plans including risk assessments in order to assist the client to progress with their recovery and rehabilitation.

  • • The Case Manager assists the client to achieve better outcomes, regain and maintain an acceptable quality of life. They will consider all aspects of physical, intellectual and emotional wellbeing. They will act as an advocate for the client if required. They are the trouble-shooters.
  • • Case Managers have the flexibility and resourcefulness to solve problems as they crop up... and the experience to forestall most problems before they occur.
  • • The Case Manager will assess the client’s needs in relation to the injuries and disabilities that they are faced with .
  • • The Case Manager will design and implement care and support programmes to improve independence and quality of life.
  • • Offer support and education to families.
  • • Provide expert assistance with welfare benefits.
  • • Attend medico-legal meetings and conferences.
  • • Provide witness statements, updated reports and court attendance.
  • • Provide client advocacy – assessing and representing the client’s best interests.
  • • The Case Manager represents the client, supports their carers/family and acts as their advocate.
  • • The Case Manager recruits, co-ordinates and manages the entire care team, training and supervising staff as required.
  • • The Case Manager ensures that the client has the necessary input from relevant medical practitioners, therapists and other professionals e.g. Architects, financial advisors and others as appropriate.
  • • If necessary, they liaise with the Court of Protection, the Office of the Public Guardian (OPG) and Court Appointed Deputies.
  • • The Case Managers are trouble-shooters and facilitators – which can mean anything from overcoming an educational or employment obstacle, to finding and converting a suitable home.
